Filming and photography consent enquiry form - Bradford Media Museum.

As a part of my unit 2 research I have been assigned to conduct interview into the impact of film festivals. I have completed this consent and enquiry form with the hope of filming in Bradford Media Museum.

FILMING AND PHOTOGRAPHY ENQUIRY FORM
The Museum welcomes filming/sound recording and photography by prior arrangement only. All proposed projects will be assessed in terms of their relevance to the Museum, their impact upon the museum’s core activities and public duties and the availability of press office staff to supervise the work.
No filming/photography can take place until the press office has received and agreed full details of the proposed project, signed copies of the risk assessment and terms and conditions and copies of any additional requested information. We recommend that you undertake a recce visit to familiarise yourself with the museum and ensure that we can fulfil your requirements.
Due to the large number of requests received, the Museum reserves the right to decline permission for filming/photography, to charge a fee and/or restrict the length of the filming/photography. Fees/timing will be agreed on an individual project basis.

Yes I would and whenever would be ok for me.

What are the proposed date/s, time/s and duration of the filming/photography?

 The filming would be me conducting interviews in the national media museum for approximately 30 minutes; the date would be any date that is ok to film as I am flexible.

This is the brief from Bradford College my place of education.

OPTION 1: Fiction Film Production
Part 1 (Unit 2):
Research: You will conduct research, using a number of different sources - internet, libraries, archives, interviews, text books, etc. (Unit 2: LO1), into the role of the film festival within the Film industry and the types of films that are selected for screening. You will ensure that the research is accurate and valid (Unit 2: LO2) and present your findings (Unit 2: LO3).

This requires me to conduct interviews about film festivals and I would like to conduct interviews inside of the media museum.

This would be a project which would be shown to my tutor Mark Duggan.


What is the proposed date of broadcast/publication and where will it be broadcast/published (please provide the geographical location and circulation/audience figures)?
The proposed time that this will be shown to my tutor is before the end of the semester in June and it will be broadcasted to my tutor in class, the audience will only be my tutor and perhaps class peers.
Please outline the full filming/photography schedule (including full details of objects and locations to be filmed/photographed):

The schedule for my filming would be conducting interviews inside of the media museums main lobby, this would include filming staff members and filming inside locations of the media museum for the purposes of research.

What equipment will be used (please provide a full list, including vehicles and any special structures e.g. lighting frames/towers)?:

The equipment will just be me with a handheld canon camera and will not include rigs, lights or and boom mics.

Yes, I will credit the national media museum in the credits to thank them for their contribution to raising awareness for film festivals.
Will any lighting and special effects be required?

No.
Do you require interviews with staff? Please provide full details of who you would like to interview and an outline of the topics/questions you would like to cover.

Yes I would like to interview any staff members that are willing to be interviewed. The questions I will interview them will be on film festivals, I will attach a copy of my questionnaire so you can be aware of all the film questions.
Will you require access to any of the Museum collections that aren’t on permanent display? (Please note that we cannot guarantee access and any requests must be approved by our collections team in advance).
I would like if possible access to any memorabilia of film festivals or promotional photos of Bradford international film festival.
Insurance is the responsibility of any crew/individual who films or photographs at the museum. Please supply the name of your insurance company and the policy number.
I am currently ensured by Bradford college but if I am required to take out personal insurance then I would pay any amount to do that.
